It starts with listening, not checklists
Every Fernhollow plan begins with a free in-home assessment. A registered nurse spends about an hour learning routines, health conditions, medications, mobility, appetite, sleep and — just as important — preferences. Morning person or night owl? Tea or coffee? Which radio station? The details that make care feel personal are gathered first, because they decide whether a plan actually works.
What the plan covers
A finished plan spells out visit times, the caregiver’s tasks for each visit, medication schedules, meal guidance, mobility notes and emergency contacts. It also names the primary caregiver and a trained backup, so there are never surprises at the door. Families receive a copy written in plain language — no jargon, no fine print.
It changes as life changes
Needs shift — a fall, a new prescription, a long winter. That is why every plan includes a monthly review: visit notes are compared against goals, and hours or services adjust up or down. Many families start with a few hours of elder care and later add nursing visits or therapy sessions without switching providers.
